Check the bolt between the intermitant shaft and the gear box. I had that problem and that was the issue.

Hello! I've been through numerous threads on this forum related to my front end rattle / knock which I hear when going over small bumps and feel in the steering wheel. Took a whole day to film parts of my car from different angles and determined from the footage that the ENGINE is what's shaking and making this noise. Potentially an engine mount? Would be the driver's side mount underneath the inverter. I don't know if this was part of a recall, will probably eventually contact Toyota and see if repair was done on my vehicle (purchased 2019).

Anyway, I compiled a video of the various useful footage. You can see the struts and all the various steering components, the intermediary shaft (one bolt was loose on my lower linkage and tightening did seem to make the steering wheel feel a bit better. I even took the rubber boot off the rack and pinion to check the bolt in the engine bay. ). If you're having this issue perhaps this post will help you with the diagnosis! And if anyone with more knowledge of these cars than myself can tell from my footage exactly what's wrong--and if the issue is worth repairing at 205K miles, what it might cost, etc--I and others will be appreciative as well! Posting this in a few of the more prominent threads so that it can be found for that person whom it will benefit.

Although the engine mounts may be worn, I would say the priority should be to replace the front struts including the top mount bearings. It is also likely that the sway bar bushings have worn. Great video, BTW.

I have an ‘09 Prius with 405k miles. She’s been an absolute workhorse but she is throwing p0300, p0301, p0302 codes. It started as just p0301 and then I tried a bunch of things and now it’s throwing the p0302 occasionally as well. When driving under normal circumstances, it sounds normal. However, when I start the car up cold, sometimes, I have a rough idle. Also, when I’m sitting at a stop light running on the battery for let’s say a minute, when the engine kicks back on, the rough idle pops back up for a couple seconds. I’ve tried swapping the coils, replacing plugs (denso iridium), replacing fuel injectors, cleaning the TB, cleaning vvt filter and solenoid, seafoam injector cleaner, etc. It definitely burns oil but the oil doesn’t look milky like a blown gasket.
Any thoughts or recommendations would be greatly appreciated. I’m a bit of a fan of yours by the way, you’ve been a great resource over the years!

I suggest you check cylinder compression. It is possible that one or more cylinders are worn out given the extremely high odometer reading.

Ok, I just ran the cylinder compression test. This is my first go and so I think I used the incorrect procedure as I did not have a battery charger hooked up. Additionally, I did not release the pressure from the pressure gauge until I switched cylinders. In Techstream, I used the compression test option under hybrid control. Do the following results tell you anything or do I have to buy a battery charger to conduct another test?

    Test 1 - Cylinder 1: 100 psi
    Test 2 - Cylinder 1: 125 psi

    Test 3 - Cylinder 2: 95 psi
    Test 4 - Cylinder 2: 120 psi

    Test 5 - Cylinder 3: 87 psi
    Test 6 - Cylinder 3: 120 psi

    Test 7 - Cylinder 4: 80 psi
    Test 8 - Cylinder 4: 105 psi
    Test 9 - Cylinder 4: 120 psi

I'm not sure if this is readily available information, but I used a compression tool borrowed from Autozone. They just temporarily charge your card and when you bring it back they refund the full amount.

Hi Patrick, why do you think the front struts are problematic? Yes I could totally see the top mount bearing potentially being an issue. I can see a gap up top where they turn, they don't seem to be excessively worn, although the sleeves are obviously falling apart. I also inspected the sway bar bushings. On both sides they do appear to be completely intact, no significant wear on either external rubber edge. Thanks for the compliment!

An inexpensive USB endoscope finally arrived from China, and yes, that engine mount appears to be damaged, will research how to replace, do y'all think this would explain the excessive engine/transmission rattle in the video above?

The compression test results seem good.

With 205K miles it seems very likely the front struts and rear shocks are worn out.

After driving it more, my symptoms have been eerily similar to this post:
Is it really an internal coolant leak? | PriusChat

I used some Bar’s Block Seal Liquid Copper as it appears to be a small gasket leak. So far, the behavior has stopped and it’s been a couple days.

I have read that the main ingredient in this Bar’s formulation, sodium silicate is some pretty powerful stuff which hardens to a rock when heated to above 120 degrees c and there are instances of people driving for years with gasket issues. I will update the forum with updates on how it goes but thanks so much for your help.

Sounds reasonable. I haven't had the car long enough to know if they were replaced beforehand. That engine mount doesn't look too good though! Assuming if the rubber is cracked like that there's a potential problem...as isn't that rubber how the motor is secured to the engine bay?

Just found this thread when researching. Will have to watch all of them with cameras and see, this does seem to describe the problem though: Post #2

Please let us know how you went with these problems. Did you replace mount or strut parts ?
