That would say, as is usual, that your 12V battery, or the connections to it, have a problem. If the parking lock mechanism was really stuck or damaged, then there would be a consistent code (P3102-xxx). But if it is 12V related, it can be transient and not stored once the car is in ready mode.

I had replaced my 12v thinking that was the issue. The car drives fine with no issues. It's strange.. and only happens when I drop to 3mph. I can drive from 1-2 and it won't appear.

Have you measured the actual battery voltage at the jump points under the hood, with loads on and off? This is the only objective data. Over the years we have seen sufficient 12V replacements that were not charged, or defective, or had loose connections, that verification is necessary.

I'll check the voltage at the jump points as well as the connection points on the battery

My issue was the battery pack and the 12v battery. Goodluck

Go to auto zone and get diagnostic test but make sure codes are cleared first. My problem ended up being the battery pack and the 12v battery. Goodluck

the hybrid battery is made up of many individual cells, and some refer to it as a pack. like a six pack.:)

My 2005 had this exact problem all lights on the dash were on. The MFD was giving the "Transmission lock mechanism is abnormal"
and the red car with the exclamation point and "problem". The ICE would not start and the car would not shift into drive or reverse only into neutral after numerous tries and it would also hold no codes. So I pulled the 12v and it only had 9.5v. I pulled my optima out of my 2006 and put it in the 2005 and that solved everything. When the 12v in the 2006 needed replacing none of this happened but the battery was at 11v. It's amazing the issues one little 12v can cause.

It's not showing any codes. Should any OBDII work?

Having the same issues. Following the advice of the threads, I changed my battery after getting the "problem with the p lock mechanism" error with the red triangle. I replaced the battery and drove about 40 miles before the triangle popped up and the highlighted vsc and the yellow exclamation with it too. The cars cruise would bot work after that but I was able to drive to the dealer. The codes i got were - p3102-581 and c2301 with 40 and 50. Dealer wants to keep my car on monday to diagnose the issue. Help!

If this is already at the dealer, you can leave it up to them to help you find the problem.

yeah that makes sense. Lets leave it for $700 worth of trouble shooting and then parts and labor on top of that.

The codes were given by the dealer. I took the vehicle in when the problem occurred.

They said they will need it most of th day so they can find the cause. 161k. I did check the battery this morning and it was 12.1 after sitting all day. I turned on the headlight, dome lights, and wipers and it dropped to 11.5
