I had this pop up after a 20+ mile drive, on the return trip back about 8 miles in. The car runs fine, gets the usual mpg I expect for my temp and driving habits - over 41mpg. But it comes back after I clear it out with my cheap bosch reader, at first it came back after a couple trips, then it started lighting back almost immediately or a few minutes or miles after starting it every time. I posted on a similar thread the following:

THEN, on the car with ! in it, i remembered that it popped up AFTER I'd last pulled my HV battery safety disconnect in trunk!
I just took it back out, CAREFULLY squeezed (and scraped the inside contact area) the little metal contact tighter without breaking plastic orange holder, and firmly reinstalled it. I'll repost if MINE comes back, as I've been reading this forum for a week looking for an answer and there's very little info on the 'hybrid system warning' alarm.
Dang it, my Car + ! came back after one trip, when I got back in and started it from grocery store 3 miles away. Anyone know a free place I can get hybrid codes scanned? I have a buddy who knows a guy but not sure yet on that. Can't afford my own scan laptop setup yet. I get no P codes, and Oreilly's scanner can't read them, or I don't know how to use it correctly.....

Bought this a year + ago with 8 bad hybrid cells. I replaced them with gen 1.5 foil-clad cells. Been great, also replaced: rear drums and shoes, 3 tires, oil changes, iridium plugs, transaxle fluid last week, pcv yesterday, a few bulbs - upgraded brake bulbs to LED's, new 12v aux when purchased, old one was bad, also k&n air filter for ICE, and misc. other non-performance items.

The 'guy' doesn't have a prius scanner, so I'm clueless. I did notice that I have minimal looking turbulence in my inverter coolant res. Maybe that's It, but I'd rather get some codes and not fix it if I'm just guessing. Following the 'who should or should NOT buy a prius', I have done all except the two coolant changes. I even tried bleeding it but probably a waste of time unless I drain and refill first. BTW, Oreilly's Bosch scanner had a lot more functions on it than my cheapie "P" code reader/resetter
