Yesterday while bucking a 30+ headwind on the highway @ 70mph, hit a pothole, got a check engine light and engine died. Traffic heavy and dangerous so I stayed on electric power longer than I should have to get off onto a service road. Determined my 12volt battery terminals were very loose from vibration, I'm assuming. But now the car will not stay running when started. SOC shows 2.5%, main battery 0volts. Towed home, recharged main battery to 7.3vdc per module, yet car still shows same SOC and will not stay running. Currently, disconnected 12 volt battery overnight to see if things look better after system sits disconnected for several hours. Any ideas if this does not do the trick?
If the 12V battery was loose, it could have very well not been charged... Check the 12V(typically called the auxiliary battery) battery's voltage. Odd errors occur when the voltage is low.
Thanks for that note, I should have stated, I precharged the 12 volt battery at the same time I precharged the HV Battery. It was very low, showed 11.8 volts when I finally got the vehicle home, and started troubleshooting/recovery mode.
Where any codes thrown when vehicle died?When you try to start vehicle do you get red triangle??How big of a pot hole did you hit I have somewhere on here before that because of Prius ride height people where damaging the transaxle from bottoming out.
If you still have a problem this morning, perhaps the problem is that the HV battery did not get charged enough. What are the individual module voltage readings this morning? How did you do the charging anyway? Ideally, you'd put in 2 or 3 amp-hours, which can be straight forward but tedious and time consuming.
This morning voltage was showing up and 58% SOC, but still it starts, then dies. After a couple of tries, it's down to 44%SOC. Will precharge again after work, I have a power supply we built up of 7ea 48volt LED pwr supplies, wired in series, to get a high voltage, low current output. Works nicely, but it does not shutoff on it's own, so it has to be monitored. I will have to measure each module individually after the pack is at rest, and get back to you on those results. I wish I could remember how to get the car in "Service Mode" or to force the engine to run. I tried the turn the heater on full blast to no avail.
Is service mode the same "dance" us gen 2 and 3 owners do?... (ignition on, gas pedal depressed 3x, neutral, gas 3x, park, gas 3x, start.)???
Okay, so you need to troubleshoot an ICE no-start. What you describe is consistent with the engine being spun but not firing. Having that HV charger is very handy indeed! Try checking the usual suspects: Throttle plate stuck shut, MAF sensor connection, pressure to the fuel rail, etc.
Precharged the 12volt and HV battery (Resting HV Voltage at beginning was 271vdc), Couldn't not get measurements of each module, but scanguage was finally showing a battery voltage and soc. Engine certainly starts, and if I'm fast enough, can put it in drive, clear the display and see that all arrows, from the engine, electric motors and battery are feeding the wheels, never does it try and force charge the HV Battery, then the engine dies. P3190, P3191 , P0000, are the codes I get on the Scanguage II, plus Triangle of Death, Check Engine light & BRAKE light on the dash. Plus the gas tank is full. Last average mpg was 59, when calculated from fuel put into tank, vs odometer reading. Throttle valve turns easy when car is off, MAF connector good, and very clean, have no way as of yet to check the fuel rail pressure. As a heads up, for awhile prior to this catastrophic failure, I would occasionally get the code for "Bank 1 to lean" if that helps any. I really appreciate all you guys for scratching your heads about this for me.

LOL, Reason I say it's running, is the students made me put a glasspack on the Prius, to make it a little more manly, and I swear it sounds like it normally does when it's running. But I will have someone or myself monitor the tailpipe next starting attempt to get a second opinion..... Interesting read on that TSB Thread. It certainly won't hurt to clean the TB, will try and tackle that tomorrow after work. I suspect, I'd smell strong fuel fumes out the tailpipe if it's merely turning over, and the injectors are spraying fuel into the cylinders, fowling the plugs?
I believe there is a bug in SGII because it should not have displayed P0000. I suggest you try my mode $13 command to read the DTC. Thanks, I will get those programmed into the ScanGuage II, and I had power washed the air filter about 2 months ago, seems clean enough when I was looking at the butterfly valve in the TB yesterday.
Does it feel or sound rough? Have you checked the ignition coils? They are the most fragile part on the engine bay. The car doesn't really throw codes when one goes out.... It just stalls or crawls like a turtle. I'm wonder if you're hearing the ice fire or just puff air since the motor turns it before any fuel is added. Does it run in idle? (park). If you do shift it in gear, can you hold the brake and gas to force a recharge? (try full throttle) the system only takes so many failed attempts before it shuts down (failed firing on ice for example). Some things seem to require restarts (several) and some a 12v battery disconnect. When I flooded my ignition coils and drained the battery down, I did about 20 quick starts (ready mode till the engine starts to turn then off) before it would start. Point being, "flush" the system of full failed starts... May it be the fuel, battery or whatever. Without a series of failed starts, and just attempts, the car seems to strive for a normal startup. I've had this happen in other cases too. Since you have a hybrid battery charger, you can worry less about killing the pack. My pack was at 1 bar and during a full throttle (sounded like mg's were overreving) it took a good minute to get to 2 bars. Make sure you're pack is well charged before doing this many partial starts. It's a lot of power going out to turn over the ice at 1250rpm or so without fuel being. Just my random thoughts.
It LIVES ! I cleaned the throttle body and MAF sensor as you guys suggested, lots of oil puddled in the manifold, still did the same thing, but this time I verified you were also correct in that the engine really was not running, for the fumes of unburned gasoline exiting the tailpipe and it never getting warm. Well, I though, why not go ahead an replace the plugs, since I had a set for the other 2002 prius, and figured sure couldn't hurt. Well guys that seems to have fixed the problem. It ran a little rough for a little while, from all the fuel I apparently dumped into the engine, and carb cleaner but has since smoothed out, Voltage and SOC are back to normal. Looks to be the OEM Denso plugs @ 158K+ miles Now all I have to do is get up the courage to take it out on a test drive. THANK YOU ALL, for all your help getting me through this problem. Great story, glad you got it figured out. For the post-mortem, what do you think happened? Could the throttle plate have gotten stuck shut when you hit the bump, and then the plugs got gas fouled?
I suspect multiple factors, first the loose 12volt cables, bump killing the engine, along with running the HV battery down to nill, trying to get to an off ramp away from heavy traffic, then for several days trying to start the car, fouling out the plugs making the already worn out, carbonized plugs even worse. If I understand correctly the 2001 electronic module doesn't wait very long before going into the no start fault, where that time has been adjusted for the 2002 and 2003 models. You can bet, I'll be changing out the plugs on the 2002 Prius very, very soon, and cleaning the throttle body.
