have any of you ever experienced or heard of a failure of the park button?

or have you exprienced an expectation that the prius would be in park, but the park button has not engaged, allowing the prius to unwittingly roll?

    Nope.

The Park function won't work if the car is still in motion. That's on purpose so you don't FUBAR the park pawl. It's actuated by an electric motor.

If this has happened to you, were you at a complete stop? Foot on brake? Press the Park button and see light turn on?

I personally always use the parking brake as well. After all, it *is* called the "parking brake."
