I have a 2004 gen II and want to know what's happening under the hood, I also suspect my hv battery is dying. Can anyone recommend a obd reader, ideally one that's an app on my iPhone and a Bluetooth obd connector. Thanks

Torque for Android or a Scangauge II are your best options. Apple has bluetooth locked down on their devices, so you have to use a wifi OBD scanner. I'm not sure what the app selection is like, Torque is the only one I know of that supports custom PIDs to display the hybrid stuff.
Engine Link is the app for the iPhone or iPad. As stated above, you will be better off with an OBDII Wi-Fi device than a Bluetooth with the iPhone. I just purchased Engine Link for my iPad from the Apple App Store and am waiting on my OBDII Wi-Fi device and check it out. Engine Link has a long list of custom PID's for the Gen II like yours and the Gen III like mine. I am currently using the ScanGaugeII which works really good and has a lot of custom XGauges for the Gen II like yours.
