Help with new battery issue? Getting hot and low charge.

About 6 months ago, I reconditioned my battery pack. Replaced 5-6 modules, bus bars and nuts, and reconditioned the rest and then balanced. Everything has been fine until today.

Today, after 1 minute of driving all of the warning lights came on and I had codes P0A80 and I think 3 for battery temperatures. The battery had a very low charge also. 1 bar.

I checked in Dr. Prius and everything looked normal, so I cleared the codes. I thought maybe a wiring harness got a loose connection for a moment because those were the same codes that came on when I forgot to plug in my wiring harness before.

I drove around with no problems, then parked for an hour. Got back in and my battery was low again. Then checked Dr. Prius and the temperatures were high, ~120f. No codes came back on.

I turned the fan on max speed with Hybrid Assistant and drove home. Temps stayed around 113f with the fan on. For some reason I never hear the fan come on ever unless I turn it on in Hybrid Assistant.

I've attached a screenshot from Dr. Prius, this is after having the fan running and everything looks normal again.

Not sure what to do about this. Voltage diff is within spec and no particular modules look bad.

Any ideas?

Thanks. It came back on today. About how long until you notice the secondary code? I drove about 50 miles today and didn't get anything new. I didn't get one last time but also didn't drive it around.
