Engine still sharking/Rattle After Cold Soak with parts replaced

    Hi All,
    First, thank you All for the Inputs, advices and recommendations on my 2012 CT200h which using the same engine chassis on 3rd gen prius. I also apologize if any inconvenience caused when I posted here, because I actually can not find the answers for my concerns anywhere except PriusChat forum.

    Let me brief it again on my issue:
    I've recently done bunch of maintenance and replacements on my 2012 ct200h F-Sport. BUT got the issue that it's shaking at start up and every time the IC kicked back on after EV mode. It's did improve after replacements and changing below but not totally resolved or fixed.

    My CT just passed 201.xxx miles on it and here are the list I've done myself:
    1. Replaced 4 new Iridium TT spark plugs
    2. New intake manifold, part#17120-37054 (I run really heavy on my CT everyday and would like to have backup parts to swap and clean the other day later). After I've replaced it and also found that it's not latest intake by Lexus TSB The latest one is #17120-37091 and you can buy it from online Toyota dealers which is fixed on MIL "ON" DTC P2111 and P0A0F After Cold Soak under updated T-SB-0103-12.
    3. And because of not latest intake manifold replaced, I've installed an Oil Catch Can on my CT too. And replaced new PCV valve when my hand down there too.
    4. Replaced new EGR valve, part#25620-37120 (ref: L-SB-0015-16 published Apr 01, 2016)
    5. Cleaning EGR cooler w/ part cleaner before put them back.
    6. Flashing to upgrade ECU Calibration ID to latest Cal-ID#37601400 (I've bought 2 days of professional diagnostic subscription with Techstream and used Global Techstream Plus, with a Mongoose plus cable for Toyota and a windows 11 laptop).     7. Replaced new engine water pump + coolant flush and change (not sure if coolant thermostat needs to be replaced too or I can keep it if it's still work?)

    AND my big concern is engine sharking/Rattle After Cold Soak still exists even it is much improved from previously Stage. Please advise what should I need to check in this case? my thought this still happen because of an old/not clean EGR cooler?? very annoyed so far. I really appreciate any inputs or advice on my case and you guys have a wonderful day

    Yet another thread started? Anyway, head gasket more’n likely your next replacement, and where your at, you should have started with that. 200k is way late in the day, about 150k late for EGR and intake cleanings.

    From your experience, would you mind to recommend a good head gasket set/brand to use in this replacement? I will try my best to have it replaced if possible

    Dang I thought you already replaced the head gasket in this venture sorry for that. Anyway so if you're going to do this now you need to get the head off put each piston at top dead center and check your deck height make everything looks uniform in the same and all of that because this takes a toll on the reciprocating mass at this high of mileage this work is hit or miss really but good luck with it I've not had good luck with any Gen 3 so far personally not been able to log more than about 60,000 mi without trouble starting and finishing and what have you I hope you have better because this is not expected of Toyota at all.

    Just continuing in a previous thread is the best ploy; keeps all the info together.

    Full disclosure: I've never done a head gasket, on any car, but I don't think you can go far wrong with the Toyota gasket "kit" which has head gasket plus most every other pliable seal/gasket/o-ring. I get the sense there's not much documentation with it, explaining what goes where. Also, replacement head bolts, while not necessity, are a good stragegy, in particular for DIY'rs who are hoping to doing it just once. Part no's for both are in the attached. I'd also check with dealership; there may be newer numbers for them.

    Appreciate it's a Lexus, but the engine "may" be identical, as far as part no's for the aforementioned. Again, check with dealership.

    Hows the coolant level in the reservoir?

    How long have you had the car?

    Any chance someone's put a stop-leak product in the coolant?

    Before diving in it'd be prudent to boroscope inspect the cylinders, preferably with the engine coolant circuit pressurized, looking for evidence of coolant leaking past head gasket into cylinder(s): tears of coolant on the wall, pool atop piston, exceptionally clean piston top.
