Headgasket was replaced about 45K ago, along with various other services- EGR cleaning (although I didn't inspect that particular work), most filters, spark plugs, etc. Car has 305,000 and runs rather nicely except for this. I started noticing it and maybe thought it was my tires out of balance or something- but then playing around with a scanner app that shows RPM's, I have diagnosed and trialed my assumption many times- it runs a little rough at that RPM when warm. I was going to give the system a cleaning- EGR system a cleaning; as referenced in many place like here:

Unless there are other recommendations? Thanks in advance.

One thing I noticed, this scanner app indicates a misfire occasionally, but I haven't investigated what it is reporting. I have a scanguage 2 I can hook up. There has been no check engine light come on yet. I guess as long as the car runs fine, gas mileage is anywhere from 45-50 mpg, could one just let it go until the misfires increase or other performance is compromised?

For me, the first things I’d check is the spark plugs to make sure they aren’t counterfeit. If they are they’ll be rounded nubs. Then I’d check the intake manifold to see if the EGR runners are clogged. Both those are common issues that cause rough running at low/medium loads.

After that I’d try using the misfire data from the scan tool and swap coils to see if they move.
