2014 Prius 19k miles P0A0F P3190 P0401

    Egr valve was already checked along with the tube and ports as far as the pcv I'm beginning to get to the point of pulling it

    I've had it to the side working on others the last 2 days I think I'm going to pull everything.... air box, throttle body, intake, egr, pcv valve, all hoses, 02 sensors, and check the whole harness....... its gotta have an issue somewhere and I've already went through most of it already but surging and roughly idling has to do with insufficient air/fuel ratio and it drives too smoothly with no issue once you get to driving it. It's only doing this when shutting off and restarting

    But they are not rebuild issues

    may help at least give you some clues where to look
    regarding those three codes.

    It was a rebuilt title car someone tore the line coming off the pcv. Even had oil collecting on the splash guards..... all of this for improper air/vacuum

    Perhaps I "missread" a previous post. But didn't you say you removed the intake and cleaned the egr ports,
    and the cleared out the oil? When you cleaned the egr system, including cleaning the cooler?

    I figured the pcv valve was not working properly, and/or a hose was cracked. I'm very happy you found it!
    And that you were able to repair it. Bet it runs great now doesn't it? :)

    Yes but unfortunately my mistake was not pulling the pcv the hose was cracked right on the fitting...... lesson learned

    Not a biggie. We've all done stuff like that. "Assuming" is never good. :)
    It pisses me off when I find something simple like that, that I KNEW I should have known....

    Just be glad it wasn't worse....

    Many if not all hybrids behave like that. One error detected can sometimes trip several or many unrelated codes. So keeping track of hard to find solutions to multiple code conditions can be tricky and well worth logging if the shop repairs a lot of them.
