Rattle on cold start, cyl 4 misfire

    2013 Prius with 170,000 miles. I have owned the car since new. I have a persistent cylinder 4 misfire, with the rattle, on cold start ups. EGR and intake were replaced with new at 140,000. EGR tube and cooler cleaned also but I did not get the software updated. When the rattle started I replaced the plugs and coil packs with new Denso parts. Still rattled. Replaced fuel injectors with new Denso injectors, still rattles. Finally replaced the head gasket with a new Felpro gasket, still rattles on cold start. Car runs fine otherwise and gets 50.2 MPG. Every once in awhile I will get a check engine light for the cylinder 4 misfire. Any help would be appreciated. It’s not a huge issue but I’m a bit annoyed that all my hard work has failed to correct the issue. Great car and I love the dash setup with the horizontal bar for the gas pedal indication!

    It could be blow-by condensing in the intake manifold. Have you considered a catch can? Are you losing coolant?

    I was losing some coolant before the head gasket replacement. Here is a pic of the cyl head. There was some black sticky deposits on number 3 and 4 cyl head.

    I’m not sure how to do that. Can you recommend a thread on checking the wiring.

    Did the rattle cease at all after head gasket replacement, and at roughly what miles was that replacement?

    I do not see anywhere on the 1st post that he checked the electrical wires, that
    connect to the coils, and injectors.

    Where did you get the spark plugs? From Toyota, or ebay or amazon?
    Or a parts store?
    You could switch the #4 and #3 coil packs,
    it would be easier to do that, and see if the misfire moves to the #3 cylinder.
    If it does, it’s the coil pack. If it doesn’t move, but you still get the misfire, then
    it’s the plugs.

    I changed plugs when I changed the head gasket. They didn’t have the Bosch plugs I
    usually get so I “settled” for the ngk iridium plugs because that’s what they had. They
    were made in thailand! Better than china, kinda.

    Somewhere after around 55,000 miles on them, I started getting a misfire here and there
    on #4. I noticed it seems to pop up when excelerating to pass another car.
    So I switched 3 and 4 coil packs. And the misfire stayed at #4, so I knew it was the
    plug.

    About 2 weeks ago when I pressed the pedal to the floor, the car stubled and the check
    engine light started flashing, but went off after I slowed down.

    I went to autozone and got the Bosch plugs, installed them and the no more misfire or check
    engine light. And the engine runs smoother.

    Counterfeit plus are everywhere!

    My Gen 3 would rattle and quit occasionally, usually in cold weather. I just tried it again, and it started fine. If you have a persistent problem, you should get things checked and perhaps a tune-up.

    Shocked no leak down test was done. Any shop can say “head gasket was changed” but not do it. That’s step one for engine knocks, if that doesn’t pass then trouble parts one part at a time.

    Thanks for all the insight! No parts were purchased from eBay or Amazon, there are too many counterfeit parts to trust those sellers. I’m doing all the work myself so no auto shop has been involved. I figured replacing all the parts that I have replaced needed to be done anyway because of the mileage and I was good insurance updating the parts anyway. I would be interested in checking the wiring if someone can provide a link or a copy of the wiring diagram. Thank you in advance for the help!

    Plugs, coil packs, and fuel injectors are new Denso parts. EGR and intake were from Toyota.

    No, can't do that, don't know of any. Do a search on ebay, I'm sure they are out there.
    But you can look at the connectors, check the pins to see if they are bent, or the plastic
    on the wires are cracked or broken.
    You can use an ohm meter also to check for broken wires....
