We are second owner of 2011 Prius dealer maintained with 127,600 Miles. Owned the car for about a year and put on about 10,000 miles ourselves.

Daughter heard a rattling noise from rear and got a check engine light. We received a p301 code for cylinder 1 misfire. Coolant was a little low but I am not sure if that was where it was before this problem appeared. There is not any coolant on the oil dipstick.
Went to Toyota website for Original Owners service history and saw that the Plugs were changed at about 97,000 Miles. I removed the Coils and Plugs and they appear ok. I moved coil pack and plug from Cylinder 1 to Cylinder 2 to see if the misfire would duplicate on 2. . I did not start the car and went ahead and did the following:

I decided to clean the EGR Cooler , EGR Valve and Intake. I followed all the steps from Nutz about Bolts. I also added the Oil Catch can and new PCV valve. I was pretty careful to make sure that everything was dry and that very little carb cleaner got into the valve area. Then wiped it all out. I then carefully reassembled , connected hoses and plugged connectors back in.

I started the car but it did the death rattle for the first time and shut off. I tried again and the car ran horribly for a few minutes then shut off and the Check Hybrid System Warning came on. I realized that Stupid me ran the hoses from the PCV and the intake for the Catch can but never connected those hoses to the catch can. So when I started the car the pcv system was not sealed and would have been sucking air.

I have now connected the Catch Can and the hoses.

I still have a Check Hybrid System and can not start the car. I do not know if I should just figure out how to clear the Check Hybrid System Light and try and start it again since the Catch can is now properly connected.
Also the car was not started for 2 and half weeks during these repairs, so the 12volt could be a little low.

Any advice on what to do next would be appreciated.

the check hybrid system should present a code, but you would need a prius aware scanner.

start it if you want to, no damage will be done in a few seconds.

Some posts had said that a little bit of rattling will happen after intake cleaning put it will clear. Is that not true?

P0301 with a 3rd gen prius is usually head gasket failure, at the wall between cylinders one and two, on the exhaust side.

If you resolve that be sure to clean the EGR system every 50k miles from here on. Did you get the EGR passages in the intake manifold as well?

More info in my signature. If you're on a phone turn it landscape to see signatures.

I did get the Intake passages yes. I was aware that it could be the dreaded head gasket. The car was running great before I took it apart and now that I have gotten it back together It rattled and now check Hybrid System.

Make sure you charge the12v to full and then try again. You should delete all the codes and then try to restart the car. Any existing codes will come back.

You need to go back and check EVERY connection.
If it was running before you did the work, then you forgot to connect something.
Are you CERTAIN you connect the hoses correctly for the pcv system?

Update. Completed the hose connections at the Oil Catch Can. Removed and Reinstalled the coils and plugs Reset the Check Hybrid System warning by removing orange safety plug and the white cord fuse int he engine fuse panel. Put back and held my breath then pressed the start button. Car started right up and runs incredibly smooth with no warning lights on the dash. I then put the wipers and cowl back in and closed the hood. Daughter said what this and handed me the plastic decorative engine top cover. Asked her to pop the hood but the hood did not lift.

If its not one thing its another..

How do you get the hood uopen if the relase cable is broken or detached?

It's not detached. Just use your fingers and get it under the hood and lift up. Then spray some wd40 on the latch once you open it
