So i have been reading many forums and watched many videos. My 2014 Prius V starting having the horrible rattle at startup, I took it to a mechanic but they didn’t know what they were doing and did seemingly nothing good. A couple weeks ago I did the job myself, I removed the EGR cooler, cleaned it, chopped off the death bolt mount and reinstalled it. The car ran great and never rattled on startup.

A couple days ago the car started rattling on startup AND now from taking off from a dead stop it will rattle when the engine turns on. I don’t think it’s the head gasket because it’s not burning oil and the oil is not milky. I realized I forgot about the PCV valve, so I replaced it today. I was super hopeful that this issue would finally be over and done, but it rattled at startup and on my test drive the rattle after taking off from a dead stop is persisting. I now have no idea what is causing the problem and I really hope you all can help. Please and Thank you!

How many miles on the car?

99% sure it is a bad head gasket!

OP did the EGR cleaning two weeks ago....yesterday the startups have a rattle.

Is the rattle engine internal noise?
Is the rattle something loose or broken?

Looking for something loose may be simple, check your work.
Just perhaps it is a non-related failure, such as a motor mount or exhaust issue.
As our co-member Brian writes, well a HG, you need to step up to an engine leak down and compression test.

That's my 2 cents... good hunting.

Seconding the above: what's the miles on it?
    Presumably the first EGR cleaning?

Likely scenario: it's around 150k miles, had only a belated EGR cleaning, so engine running hot for some time, and head gasket started leaking coolant.

It's short sighted, to think that cleaning the EGR now is somehow going to fix the head gasket that's already failing.

So I’m convinced, I just called a local shop, they said they recommend an engine rebuild and to level the block. That would run 5500. He said it’s common for the head gasket to be replaced and for it to go bad again pretty quickly.
What do you all think? Have you seen this to be true?

If you can manage a DIY head gasket replacement, the Toyota gasket kit is around $200~250 USD, and a set of new head bolts maybe $100 USD. I wouldn’t assume any of the machined interfaces need machining, doubly so with symptoms just starting.

my signature has a couple of head gasket related links, and last link is the full repair manual engine section (has the various machined surface flatness specs).

How’s oil consumption? Toyota revised the piston and rings partway through model year 2014, due to oil consumption issues. The attached has info on that, or a new short block is around $2k USD (will have the revised pistons/rings).

Full disclosure, I’ve never done an engine tear down. Hopefully never need to.

???How did you reinstall the "deathbolst mount"?????
I removed the EGR cooler, cleaned it, chopped off the death bolt mount and reinstalled it.

    How many miles????

Remove the spark plugs and look into the cylinders. A bore scope works best.
Do the #1 and #2 pistons look cleaner than the other two? Are you loosing coolant?
If so, likely a head gasket. Burning oil or not the head gasket can still fail.
You won't see coolant in the oil unless you have a very serious problem. And likely won't
be able to drive the car.

If it is the head gasket, the more you drive it the more likely you'll warp the head and/or throw
a rod through the side of the block.

Obviously whoever you took it do knows nothing about the Prius. Are you close to the Hybrid Pit?
