Need to replace the engine in a 2019 Prius LE. 190k miles Blown head gasket, coolant in cylinders 1 and 2, rough start on engine, blowing white smoke, and starts overheating after about 10-15 min driving. Are all Gen4 engines the same? Can I just replace with any used 2016+ engine? Do I need to find a "Prius or hybrid specific" mechanic to do the install, or can just any competent mechanic that can swap a regular gas engine do the job?

You'll have to check the local market. I know when I test drove my Gen 4 in June 2016, the Corolla Hybrid was only new here - with a Gen 3 Hybrid engine. I think early 2019 we got the upgraded version. Do you have a Hybrid specialist in your area - I hear there are a few in USA.

I purchased from auction "as-is" and unfortunately had already titled into my name before the check engine light and temp sensor came on several days later. Now I'm stuck and need to figure out the best solution fast. I have no idea how long the car was driven while overheating and whether the head is warped. I've heard that even if the head gasket is able to be repaired, something can potentially fail again as the engine is over 190k... Is there a substantial price difference between swapping with a lower mileage used engine vs repairing the head gasket? I'm located in Orlando, FL
Not sure about price difference, for me its easier to do a head gasket than a full engine swap (if rods are not bent which hopefully they wont be!) I believe its normally rods that bend on these motors, not the head its self... i could be wrong! for me, If the rods are good i would definitely just go ahead and do the gasket on the original motor, obviously not everyone will agree that's just what i would do! then clean the egr and ad a $20 oil catch can while your there to prevent future egr problems which lead to blown head gaskets I still struggle to fathom how the thing ended up with that many miles already Good luck with it, cheers ..

No. The Electronic Parts Catalog lists a different part number (19000-37850) for model years 2019 and later. An engine from an older fourth-generation (2016–2018) model might work in practice; I haven’t tried it. Since these aren’t identical to the later (2019–) design, however, installing one would violate federal law, unless you repeat the certification testing that Toyota did: see the EPA Tampering Policy (PDF), page 7, Basis A, note v.
And transplanting a 4th gen engine (any vintage) into a 3rd gen Prius (or Prius v) is completely beyond the pale? (Rhetorical question )
Unfortunately, yes: the regulations require a combination of engine, ECM, transaxle, and chassis that has been previously certified. (See my previous posting.)
Did you already clean the egr cooler and valve? Was it a difficult task xompare to the gen 3? I have over 140k on 2016. No issues at all so far
