Did you notice the voltage readings on the first 3 blocks? 12.8v, 330v 328v

If #1 is correct, then there's definitely still an issue with one of those modules. The other 2 are obviously whacked and could be a torque setup error or the harness plug/ecu socket has some issues.

    Ha, Ha. That is hilarious.
Did not see that. I was on mobile without my glasses.
When people setup the gauges like that it is way too tiny for me unless I go grab my glasses.

Kinda worries me if OP was not aware that that might be an obvious issue. :)

    Eric:

Thanks for the great advice. I have the same P3012 code after replacing all the cells in my 2006 with cells from a 2011. I'll now re-check all of those battery post nuts, connectors and crimping.
    1) What should the torque be on the battery post nuts?
    2) Any suggestions on how to clean the connectors on the voltage sensing side of the battery?
I cleaned all the copper plates in vinigae and sale and then sanded with a 400 grit paper but did not want to risk damaging the wiring on the volatge sensor side, so any suggestions or comments would be appreciated.

If the voltage sensing wires are bad enough to need cleaning, instead you could just by a new wire frame from Toyota parts department. Comes with new wires and bussbars.

Ok so from what i was reading this kinda sounds like the same problem were having bought a brand new one from O'Reilly's spent about 1700 a few months back and now its throwing code p3012 and said replace hybrid batter pack system could there be something else wrong with the Prius causing the hybrid battery to be going bad?

What you bought was a second-hand battery from O'Reilly's and it is highly likely that it is not something else. Old batteries fail. You need to look at the warranty you received with the battery and find out how you make a claim.
