So many variables with your car that can cause this. How's your alignment, air pressure? Maintenance done lately?
    Original battery?

Hey, it's WINTER here in the North East, therefore WINTER = MAX USE OF HEATER, therefore MPG will take a DIVE!

Q.E.D. ;) Happy New Year, World!

Basically, I did change the 12v battery with a good one
Well I did replace the 12v with a good battery measuring 450 out of the rated 325cca, and usually has 12.3 volts after sitting a day or two. Is that ok? Other than that, not much else has been done or needs to be done. But, I did notice that the traction battery charges to green rather quickly, but will still pull the car up a small hill on battery alone.

Possibly. Also, my fuel bladder capacity has gone down to about 8 or 9 gallons. Could this give lower MPG reading?

no, but the best way to check your mpg's is to calculate each fill up manually

12 not great but not causing your problems. Clean throttle body, air filter. Tire pressure 36+? Brake not dragging? Tires wearing even? Use any injector cleaner in a while?
How many miles on the car?

224k miles. I did a front toe in alignment not too long ago. Like I said, the rapid decline happened right along side the dropping temps(40-60) here in San Diego. In the hot months, MPG were mid 40s. But, the HV battery is charging and discharging fairly quickly. Beginning to think its getting close....

Tires were feathering bad, alignment was needed. I run 42/40 on the tire press, although one tire has a "one week leak" and I have to refill it constantly, This along with the remote alarm not working this Prius is not as nice as it used to be...

My mileage goes down a little during winter, I'm 3 hours north of you. But only a few mpg.
My battery can cycle too. I keep thinking it's going to go but it keeps going. It's a rebuild thats been working for over 2 years. Knock on wood. Lol

Indeed, these cars are complex and the energy monitor keeps you in on how it works, but they do just keep going regardless. I need an oil change but thats about it

Wouldn't hurt cycling some injector cleaner through it on a couple tanks.

Something to note, usually when I give it a good shove at take off I'm getting about 6-8mpg, is that right?

That's what the meter says. Not correct.

accelerating from a stop is energy intensive
