2018 Prius Prime HOV Decal Application

    Hello,

    I bought my 2018 Prius Prime on 9/30/18 and received my license plates a month later but until now I did not receive the HOV partially filled application form from the DMV.
    Should I download the application form REG 1000 from the DMV website to complete and mail to the DMV? Please advise or let me know the proper procedure to follow.

    Yes, you mail in the completed application with the small fee.
