Hi all, Got a 2006 Prius a few weeks ago. Has been driving fine, no errors and getting on average 5L/100km, so not too bad fuel economy. It has done 120k kms. Just got a Bluetooth obd2 attachment and been playing with a few apps. All seem to be showing module 2 as negative 16v and module 3 as positive 26v, all the other modules seem normal voltage. Anyone seen this before? Seems like maybe a sensor error to me? Thanks for your help. Posted via the PriusChat mobile app.
Usually see it when there is a bad connection, sensor, or wiring. Did you or anyone else do some DIY repairs on the battery pack?
The guy I bought it off said he removed and cleaned all the connections, then put them all back. Said he did not remove the pack or any cells. Would you recommend opening it up and checking the connections?
Definitely. Have your torque wrench at the ready and check all the nuts for the modules and HV connections. My guess is previous work was done with a "guess it seems tight enough" attitude. Also check the sensing wires for breaks. Check the orange connector and pins at the computer for corrosion. Clearly something is wrong.

Maybe I should try it with a new battery and a failing one to see what it says. Or go to a dealership and test drive their car and see what it says. That might give you a baseline.
+ 1 to Eric's suggestions. The odd readings of one module showing negative and the next being double are usually short lived. A broken sensor wire should soon cause a P3030 code. Be careful when inspecting the voltage sensing wires. High voltage is present. Here is a thread covering fires at the voltage sensing harness. If the car is not throwing codes I would question the PID formula on your app. Sometimes there are typos in the PID definition and the reading comes out wrong, though reality is fine. What app are you using?
