    Well, according to the OP:

The hybrid battery fan was running at full blast (not a good sign at all) and the dash was lit up like a Christmas tree.

OP didnt mention any driveability concerns, but most people say when their traction battery is on the way out, the ICE runs more, noticeably less power, and the energy display shows the traction battery discharging and recharging very quickly. Good news is the ECU's monitor the traction battery like a parent with their first baby. It monitors voltage delta pretty closely and I think I read something like if a cell or block is off .1v, it sets DTC's and lights up the dash.

In hindsight, I do remember the few times I drove it recently (it's the wife's car) that the ICE was running more than I remembered, and it seemed "lifeless". I didn't monitor the energy display.

We had never reset the MPG indicator. So we could say what the lifetime mileage was. It was 49.9. Had been over 50 until set of tires #2 (not low resistance) drug us down. But even with Ecopia's it never budged.

But the MPG reset when the battery was changed and we are at 55.5 MPG in the 150 miles since the replacement.

Yup, that's a fail...except the third value is the inverter voltage and is near correct. Get the 12v battery replaced and your mpg should go up...YMMV.

Also, PLEASE change your transaxle oil. The transaxle is more expensive to replace than the batteries!!!

Looks like i have to call the dealer.
My car when it went out showed the same symptoms. The backseat vent went on. Lights went crazy and battery was showing it going on and off crazy. Is there an at home test to find out or is it a dealer only test.

You can get codes read at many local auto parts stores or your mechanic might be able to get them...post them here. However, your first step isn't to call the dealer. Your first step should be to rule out the 12v battery as being bad. If your's is still the original--it's toast: replace it and see if the symptoms improve or go away. Only then take it to the Stealership for HV battery testing.

It was replaced a few days ago, and then this stuff happened.

Apparently it was replaced twice already wasn't it. Once with a new Optima from autozone, and then again by the dealer about 100 miles later (who said that they had to replace it because it was "incompatible", along with about $850 worth of other work that did nothing to solve doomteams problem). This is according to his post here: 2004 Prius Warning LIght and Engine lights | PriusChat

If you have any options at all then please stay well away from that dealer! Monitor the performance of your HV battery for a while and see how it plays out. Look for things like very rapid changes in state of charge (the blue and green bars on the battery display) as well as the battery fan in the rear vent. Yes there are some simple tests you can do, I suggest you monitor the things I've mention and start a new thread about this. You definitely want to try and figure out as much as possible about the problem for yourself before going anyway near that dealer, because they sound clueless.

There is another toyota dealer about 2miles farther. And yes it has been replaced twice. In a single drive of 5miles the batter charge changes from fully charged to one bar above empty.

So I was jotting down a list of things I need to do. I have 12v battery, Transaxle oil (which is same as ATF I assume), Inverter coolant, and Plugs/PCV.

    Anything else?

Also, I had the recall done for the HV Water pump done in Feb 12 at 82,000 (Thanks for the link to the service history someone posted). Does that negate the need for the Inverter coolant change?

I plan to get pricing from dealer and then compare them to Mellors. If Mellors saves me enough, maybe I'll make this a Prius Spruce Up/Spring Training combo trip. (It's 2.5 hours to Mellors)

If the dealer followed the TSB on the HV coolant pump, the coolant was drained and refilled at that time. The problem is that some dealers (who should lose their Toyota dealership) will clamp off the hoses and just add some new coolant to make up for lost coolant so its hard to determine the need unless you know what was done.

You should also check into the ICE coolant as well. It was due to be drained and refilled at 100k miles. Bot loops should be refreshed every 50k miles thereafter with Toyota pink, pre-mixed, Super Long Life coolant.

Thanks. I'll add the ICE coolant to the list. I guess I'll ask the dealer about the HV Coolant and trust them based on what they say.......

I'm not sure how I can verify.... Anyone know about how much the Plugs/PCV might be?

How many gallons of SSL coolant were billed for in the records of the recall work? If less than 1, they didn't drain the coolant.

Got estimate from dealer.....so far, Mellors has not responded to my request for estimate.
