2005 Prius, 123,500 miles. It was bought in and lived its life here in California. Looks like the HV battery is mostly dead. The first symptoms were rapid changes all the way from green to purple and back, and now lots of red triangles. The 12V battery is fine. In fact I replaced that one myself about a year ago.

If it had to fail, I'm glad it did before the 10 yr / 150,000 warranty required by California ran out. My question is this: if I get it replaced under that warranty, do I get a new HV for free? Or do they prorate the cost of the battery for the lifetime it has earned? Is the installation labor free as well?

It is not prorated. It is 100%.

But the replacement battery is only guaranteed for 1 year (unlimited miles) so next time (100k+ miles) you will be on your own.

I do not know this as fact but surely provided you are still within the CARB state 10year / 150,000miles even outside Toyota one year guaranty on a new battery, the CARB state would guaranty take preference so the OP has 26,000 miles and 2 and a bit years left.
