Has anyone information on how to obtain a single, replacement CA HOV sticker? Fender repair on front right required removal of the old one. Thank you.
I'm guessing you fill out the DMV form and check the box for "replacement" stickers. http://www.dmv.ca.gov/forms/reg/reg1000.pdf
I sent in the form for a replacement on 11/10/2006 and DMV has yet to cash my check, or send me a rejection. The collision repair shop cut out the originals from the bumper when they replaced it. We carry them in the glove box in case we DESPARATELY need to use the carpool lane. Not sure it is worth risking a $300 fine however.
If you have stickers on the other two bumpers I'm sure you would be OK -- just keep a copy of the application and your check draft until you get a replacement.
I just got rear-ended, and the bumper needs to be replaced along with the HOV stickers. The application says that I need evidence of FasTrak account. Do I need to send that in since I'm just asking for replacement?

Since it was not clear what I should include with the application. I just included a copy of the Fastrak paperwork I used when submitting the original (new) application, together with a copy of the latest Fastrak account statement (month of November). I sent in my application 12/29/06 and my check was cashed on 1/16/07. So, apparently that works. Now, I'm patiently waiting for my replacement stickers. Update: just received my replacement stickers today 1/19/07. HE089971.
