They should be sending out the Notice to anyone who activated service for a V710 on or before January 31, 2005. I bought a V710 for my wife and another for myself in December of 2004 but only received one Notice and Claim Form......for my line. However, class members who did not receive the Form can print it out from the Verizon web site, complete it, and send it to the Administrator.
