I'll have the form shortly. I purchase my vehicle outside of San Jose (Sunnyvale) and live in Gilroy. The dealership responded that they had the form and were filling it out and sending it to me. The end result is supposed to be a permit sticker. I'll see how it goes. Ken

Months ago the Toyota dealer on Capital Expressway had this info in their sales ads. I thought that you had to actually buy from a dealer in San Jose to get it, though, not just Santa Clara County.
Well, no such luck. I received the form filled out by Toyota Sunnyvale. Even though the form said it needed purchase info from a SJ Dealership I figured I'd send it in anyway. Got it back today denied. I sent a note off to the dealership asking them why they are sending out the form if it's not acceptable, maybe they know something... Ken
