I bought 14 with 150k miles about a month ago. It was running great up until about a week ago where it will occasionally have a rough start for a few seconds before calming down. It may be a coincidence, but it seems more likely to happen when the car is parked with the front of the car at a higher elevation than the rear.

My obd scanner shows pending codes for random cylinder misfire as well as specific codes for cylinder 1 and 2. I know that’s a common sign of a head gasket failure so I removed the spark plugs and took a video with a cheap Amazon endoscope while I was in there. Cylinder 2 looks oily or possibly coolant-y (I’m not sure how to distinguish between them). This is the video I took. It’s in order of cylinder 1-4.

Would this be a head gasket issue or something else?

I also can’t help but wonder if the previous owner used some kind of head gasket in a bottle. I was experiencing heat issues for a while (low heat, temp getting stuck on cold - fixing by adjusting temp up and down), but that has mostly resolved itself over time somehow. The coolant in the reservoir looks clean, but maybe a little bit low - about an inch under the low mark. That’s with putting about 600 miles on the car. Also, the oil on the dipstick and on the oil cap looks normal from what I can tell.

the seller might have added stop leak to disguise the symptoms temporarily, top up the coolant and keep a close eye on it.
has the egr circuit been cleaned?

150K miles is where the 3rd gens reliably commence blowing head gasket, owner panics, dumps in stop-leak, trades-in or sells. The stop-leak product gradually lets go. Who'd you buy it from?

Just a private seller. I marked the coolant level and it does look like it’s slowly going down.

I’m super bummed out. I’ve got about $4800 into it, and I don’t think it’s worth the $2k plus to repair, so I may just take the loss and sell it as is. If I knew it was only the head gasket I might feel a little better, but it sounds like HG is usually paired with other issues like a bad coolant pump or possibly damage to the engine itself.

Cylinder one, and sometimes two, is where the failures usually start.

I typed out a tome or two, all the options, then deleted a few times. Short summary:

It’ll likely take around $3k USD to resolve the current issue, and fend off likely pending oil consumption issues (due to Toyotas decision to use low-friction piston rings).

if you stick with it, also ensure the EGR (Exhaust Gas Recirculation) system is thoroughly cleaned.

There’s also a couple more possibly pending issues: brake booster and hybrid battery failures, with similar repair costs.
