Having issues with P2195

    I’ve been dealing with this code for far too long.
    2015 Prius Two 293,000 miles
    What’s been done in chasing it down:

    Air fuel sensor, leaking exhaust manifold to converter gasket

    Code returned with a no signal code as well

    Checked wiring, took apart a/f plug to check wires and connections. All checked out good, adjusted the a/f sensor pins

    Code returned, the no signal code gone

    Engine began random misfire, code said cyl 1
    Finally thought I found it, coil and plug are ok so replaced injector.

    Misfire code gone, car runs better but P2195 returned

    So pulled the intake, cleaned it, put in a new pcv valve, cleaned the maf and iat sensors, checked the intake for cracks and all the hose connections. The gasket was oil soaked so again figured I’d found the source. Put it all back together..

    Code returned ‍♂️

    Thoughts?

    With that mileage, you didn't just replace the A/F sensor? While not a commonly reported problem here, these sensors don't last forever, and their degraded performance can cause error codes. That's why the first diagnostic in the repair manual is to check the sensor output voltage.

    By the way, has your EGR system ever been cleaned?

    Update:
    In going through all the diagnostic flow chart it was leaning towards the ECM/harness

    Last effort was to just unplug stuff, even though they passed the check list

    Started with the EGR since it’s showing it’s operating at +6 steps from where the computer wants it. More power, random bucking chugging still present, P2195 returned.

    MAF disconnected, random bucking still present, P2195 returned

    MAP disconnected, no bucking, missing chugging, 600 miles and no return of P2195

    Replaced MAP and did a preventative replacement of the MAF, both from Toyota.

    60 miles later that damn P2195 returned

    Totally at a loss, guess I’m going to have to spend several hundred at Toyota for them to dig deep into the computer.

    To answer, EGR hasn’t been cleaned, I have a new, well 15kmi, part to swap out eventually. But code returned with it eliminated.

    Yes have checked for vac leaks

    Car is on it’s 3rd Denso A/F sensor, the leaking manifold to converter gasket killed the heater circuit in 2, expensive lesson there

    Because of the "bucking" and previous misfire code, and with that mileage on an uncleaned EGR system, this is starting to sound like an unusual presentation of a head gasket failure. Have you lost any coolant or oil?

    If you're lucky, it's simply a clogged EGR cooler.

    Likely not the fuel pump as the car seems to switch to a very rich condition after traveling a few miles at 65mph+. Something is telling the injectors to go to maximum fuel spray which is causing the instant mpg to drop from 50ish to 15mpg, causing the black smoke, triggering p2195… which is somehow causing hv battery charging to stop, resulting in overall power drop, as the hv battery depletes… and car rolls to a stop. Key question is … why after cruising at 65mph for 3-5 miles does something suddenly cause the engine to go super rich and trigger the above situation… this never happens below highway speeds. Car runs fine and code free at speeds under 60mph…

    Frank that code is Bank 1 Sensor 1 too lean (not too rich). If it's happening only on the highway, then possibly it's starving of fuel supply causing a low power condition and relying only on battery which depletes quick. Usually the strainer filter on the fuel pump assembly in the tank gets clogged over time from using fuel cleaner additives and bad gasoline. Remember the P2196 is the rich version of that code. We just did a fuel pump on a vehicle with the same code and it fixed it completely after chasing O2 sensor/Catalytic converter down a rabbit hole. They are about $400 or so but it just might fix that condition and code for you. Let us know.

    You "should" do a fuel pressure test, and volume test.
    That will tell you the condition of the fuel pump.

    car drives fine for the first 3-5 miles at say 70mph ...then something has had enough and drops mpg and black smoke out the tail pipe...at this point I will consider all ideas ... so thanks and I will definitely post updates...
