Either the voltage sensor harness has been damaged from corrosion or your battery pack has gone bad. If you want more details, you need to provide more details. Like what are the symptoms, vehicle history, ownership, what's your budget for repairs and if you are DIY repair person or have a mechanic?
It means I would be inspecting the "wireframe #2" harness, plug and ecu socket on that HV battery pretty soon before it goes pooof. Blocks 1/2/3 are pretty much where the ecu pin corrosion starts
That is, in non-inside-out English, the number two frame wire. Or, the way it goes alphabetically in the parts catalog, inside out with commas, "wire, frame, no. 2."

Hello , thanks for the response, seems like the car pretty much just runs on gas not in the electric mode, its a used car 2011 , i started getting the master warning light , and the brake system warning light, check engine light, the slip indicator light all at the same time, and the check hybrid system message, also when starting the car and shifting to drive it started just going into nuetral, I usually try and fix everything I can, budget is pretty low, i have low income
This problem is more common with 2004-2009 Prius, but the good news is if this is your problem, the replacement part is less than a $100 and there's lots of videos on Youtube about how to pull out your hybrid battery pack to clean it and if need be replace wireframe #2. Sometime if you get the corrosion cleaned out before it gets worse you can get by without the new part. As for how you car is behaving, that's a default shut down sequence to protect the hybrid system. You can clear the codes by disconnecting the 12v to see how long it takes the car to find the problem again.
As you have warning lights, have you retrieved the trouble codes yet? They'll get us started on what the issues are. Dr. Prius has a reputation for not showing all the codes that the car may have for you. but if you've got the master warning light and the check engine light, I think it should at least show you the codes behind those. Codes for the brake and slip lights you can get without a scan tool if need be, this way. (There's a good chance, anyway, they'll only be the C1259 and C1310 codes, which mean "have you noticed your master warning light and read those codes?").

But I'm not really Blink Codes Guy. I'm "it's helpful to know the codes" guy. If somebody has a scan tool that works well enough, that's the easier way to get them. I'm only blink codes guy when somebody doesn't have a scan tool, or the tool's not showing the codes, and the person's stuck thinking there's no way to get them. But I'm always Non-Inside-Out-English guy. "Wire, frame, no. 2." in commissary-speak is just plain "number 2 frame wire" whenever the person speaking isn't Yoda. But wasn't there some kerfuffle about whether changing those "senior member" descriptions even works anymore?
I wanted to get teckstream so I could maintain the car without get hammered at the dealer, but I saw some selling for 1500 dollars and some certification needed and also some free down load , because I have a older prius that has been sitting for over a year doing that same thing that it just shifts into nuetral, that one the 2008 I pulled the hv battery out did the whole recondition of the battery with the hobbie charger and nothing, goes to nuetral, how do i get teckstream going? as far as codes it has p0642 change my handle ? sure i guess )
If you have a computer google "techstream in 5 min" Theres a mud member that set up and you can use free as long as you have cable.
when the first two block voltages are what is shown in the OP photos, there isn't a lot of troubleshooting required........
I don't think P0642 is a code in a Prius, but is there a chance that could be a P0B42 ? Maybe somebody wrote a lowercase b and it looked like a 6 ? P0B42 would suggest damage to the second voltage sense wire in the harness that carries the block voltages back to the battery smart unit to be measured. That's consistent with one block showing a voltage lower than even a worn-out battery block ever really has, and an adjacent block showing a voltage closer to that of two blocks. In Gen 1 and Gen 2, that wire harness is available for sale, called the number two frame wire, and it's under $100 as I recall. Disappointingly, in Gen 3, Toyota stopped offering it for sale. Now the parts diagrams just show it as "This part is not supplied as an individual part because it is difficult to keep the function / quality of part assembled". They would have you buy a whole new battery instead. So that leaves trying to source it from someone who dismantles or rebuilds these batteries, or from an aftermarket supplier, if there is one.
The code and symptoms you have are not about the condition of any blocks. They're about the circuitry the car uses to sense the condition of the blocks. So it's not that the car is telling you anything is wrong with block 1 or block 2. It's telling you it doesn't know the condition of block 1 or block 2, because the sense wiring (that "number two frame wire") or the monitoring electronics have gone bad. Here is an old thread giving some idea of what commonly happens with that wiring. The problem is often resolved just by replacing the number two frame wire, or the battery smart unit, or both. In earlier generations, anyway. But in Gen 3, Toyota won't sell you a number two frame wire. So that leaves the problem of finding some other way to get one.
