Loud Rattle After Head Gasket Change

    I made a post here a while ago about my 2012 V losing coolant and decided to put the car away for a while since I was expecting to do a headgasket change. I tried changing the waterpump, thermostat and doing an egr cleaning but it didn't fix the problem so I assumed it was a blown headgasket. I watched hours of videos on how to do it and everything seemed fairly simple just a long process.

    The part that I was worried about was the timing chain being put on correctly since I'm no mechanic but from the videos I watched there was a way to lock it in place and put it back on following marks left by Toyota and all was suppose to be fine.

    Again I am a just learning so some terms are new to me. But from what I've read and watched the engine is suppose to be on Top Dead Center which means the 1st and 4th piston is flush with the engine block? Which it was when I removed the cylinder head.

    If it matters I did have the head resurfaced, pressure checked and replaced the valve stem seals.

    When I put it all together I followed the videos I saved and lined up all the marks on the chain to the marks on the engine. I put the timing chain tensioner on (with the hook facing upwards) and turned the crank until I heard and seen the chain tension up.

    Put everything back together and started it up and I got some very rough rattling and shaking sounds and feelings on the car. Waited a few seconds to see if maybe that's just how it is at first and it just continued.

    I shut the V off and took the valve cover off and the crank wheel so I could see the marks on if the timing jumped a tooth and it did not. All marks were lined up.

    I have searched the forums for problems similar and found 2 posts. I can link later if needed but one post said that it ended up being his egr being stuck? Because when he disconnected it his car ran fine.. Another post said there was a blockage in one of the oil passages and that fixed it right away.

    Obviously I would not like to open the chain cover all up again to check for blockages but if anyone has any other ideas of what it could be I would really appreciate it. I'm going to take pics later today and put it all back together. I will check all my connections to see if maybe something was not plugged in all the way. I will also record a video to show the rattle. If we can't figure it out I will just take it all apart again and check for mistakes.

    How long did it "continue" to rattle before you shut it off? Did it rattle continuously on several startup attempts or just once?

    If it it never clears you probably have a hard plug, coil or timing misfire. Maybe a vacuum leak.

    It ran fine for a few seconds and rpms went up and thats when it started to rattle.

    I can check for a vacuum leak but what do you mean by hard plug? Also I ordered new fuel injectors and am looking for coils right now.
    Curious though on how it would be a timing misfire if I followed the instructions exactly from those videos and checked timing marks and it did not jump a tooth.

    Bad spark plug. But first things first. Did you try to start and run the car again after the first rattle?

    So I put everything the valve cover and intake stuff all back together and the rattle continued. But I let it run longer to try and get an idea of what the noise is and I don't believe it is timing. It is definitely a misfire rattle. Crazy how much it rattles from a misfire.

    I took all the plastic (that holds the air filter) off to get to the throttle body and I noticed that when the car is at a low idle and the little butterfly valve is closed the car runs fine with no rattle. The valve opens a little when at a high idle and the butterfly valve opens slightly and that is when it rattles. I can also hear a hissing sound from the intake manifold area.. was maybe thinking it was the sound of the fuel injectors spraying. But when I accelerate the rattle goes away...

    I can post a video but honestly just sounds like a typical prius misfire sound..

    Hissing sound is probably a vacuum leak. Maybe a leak between the throttle body and the intake manifold.

    You might be right. I found a loose bolt on the throttle body.
    Took the manifold off to clean it some more and to have more room to slide the new fuel injectors in.
    Will put it back together once the injectors arrive. Will update.

    We have seen pcv hoses disconnected causing trouble.

    What all connecting to the intake:

    1. Throttle body
    2. Engine intake ports
    3. EGR pipe
    4. PCV hose
    5. Gas tank vapour hose

    That's it? Not sure.

    Thanks! I will install it this way.

    Yeah I've read that before and made sure to check all the hoses. Will check hoses and nuts/bolts again when I put it all back together. I also replaced the pcv when I first go the car. Replaced spark plugs and cleaned egr and intake manifold but I guess not good enough.

    The good news is that when it's idling calmly my car isn't spilling coolant so I'm assuming the headgasket install for the most part was a success? Just got to find out what is causing this misfire.

    Other than the in/out coolant hoses and two plugs (for throttle body and maf sensor?) that's it.

    Coolant hoses and electrical plugs don't introduce air leaks though.

    Oh yes you're right, thought you were just wondering what all connects to it. I'll double check all those connections and see if I can find anything. Thanks.

    You must not have Techstream or Dr Prius if you did it would confirm your missfire if it was just one cylinder. With out software you have no idea which coil on plug COP has failed.

    I assume all the cop power plugs are inserted and clicked and locked?

    If Techstream showed specific cylinder misfire that would be great just replace that cop. If it showed an all cylinders missfire or rolling missfire then it would most likely be mechanical and the vibration of that bad mechanical issue distorts the waveforms of the cam and crank pg sensors so then the engine ecu gets confused by these distorted waveforms from the sensors and runs home to mama and throws a rolling missfire.

    Read the codes it’s throwing.

    I usually use a couple devices to scan for codes but I do have techstream and will give it a scan once I put it all back together. Will update with that information as well.

    But yes I quadruple checked the plugs for the COPs.

    Thanks for that info.. I'm crossing my fingers that it's just a bad COP or spark plug.

    I hope so too you did alot of work there. This engine does sound terrible when a cop fails. You just need to figure which one.

    If it is a coil on plug don't waste your time on auto parts store cops there half the price of the factory oem cops and don't last long at all.

    Olathe Toyota online parts.

    You have to eliminate the what ifs on this hunt. TS will get you out of the forest. Good luck.

    Update: I changed the fuel injectors, cleaned the intake manifold completely and installed it back on. But rattle continues.
    I scanned it with techstream but I think that a lot of the codes it threw were from the rest of the plastic intake stuff not being connected. Will connect all back on to see what codes it continues throwing.

    Misfire continues though, not sure from what. Let me know if these codes give anyone any clues. I will upload new images of the new codes by tonight.

    Update: Images are attached of the codes it throws now.. Everything is back on and plugged other than all the windshield wiper stuff.

    I notice you never mentioned the miles on this car.

    You have misfires on all 4 cylinders. If it was me I would try a hail Mary buy a real Toyota cop online for $100 and try it on the 4 cylinders one at a time and see if just one cop is bad.

    But i doubt a cop failed just cause you removed it. I fear your rebuild methodology may not have worked out.
