Hello, I just did Head-gasket Job on My 2012 Prius V, I had signs of Blown Head gasket , loosing Coolant, White smoke out the back and rattle but the rattle wasn't that bad and had just started when I pulled the Head out and replaced the Head-gasket, water pump, Thermostat and Tensioner, and when I put everything back together I got no check engine light and the car is making unusual rattle ONLY on Idle when i give it gas its fine and runs normally here short clip of it please let me know what you guys think

While the head was off, did you happen to verify piston to deck height on all 4 cylinders?? It's one of the best ways to check for a bent connecting rod, without total disassembly. A slightly bent rod will show up as the top of a piston being slightly lower than the other 3 when at top dead center. A slight bend will cause the piston to slap a bit. Eventually would likely lead to ventilating the side of the engine block. Just something to think about.

EGR valve can cause rattle. Toyota has a newer design and ECU reprogram.

Don't know if it applies to Canada...

    moto g(7) power ?

When I ran health test with techstream it said my 4 coils were bad and it had x beside where it’s permanent and wouldn’t let me clear the code but my check engine light is off, i cleaned the hell out of the EGR and changed all its gaskets.

That's ~180K miles, which could be bent connecting rod territory. Remove all spark plugs, place a hardwood dowel in each hole (one dowel, placed first in cyl 4 then the successive cyl's), manually cranking engine to TDC with each cylinder. Use a very very flat/even blade atop the valve cover case to put a mark on the dowel for each cylinder. Maybe make the cuts 90 degrees apart around the dowel, and label the cyl no for each with a sharp-tipped felt pen. Then check if all the cuts are level.

Very recently, after seeing signs of head gasket failure, which probably let coolant into a cylinder or two, historically likely to be #1 or #2.

Thanks I appreciate the info, I am hopping it’s just coils and injectors that are not functions correctly!

I did check the heights using straight edge and filler gauges all seem good, I am almost certain the problem is with my coils , turned the car in the morning today it ran very nice for solid 3 min and the noise came on for few secs and it went away again, my coils are on the way I didn’t go for OEM just yet but I probably will just gonna use these as test plugs and if they work out I might spend the money cause my Orginals seen better days

Wait. What does it mean again when the IC codes are "permanent" but not current?

I would buy a boroscope and check inside the cylinders. You may still have coolant coming in the cylinders.
