You need to open the HV battery covers and look at the HV computer and connections for corrosion or burnt wires, replace as needed. Replace weak module. Your catalytic convertor may be blocked with soot or no longer working.
If I open the hv battery, do I need the 12v Disconnected? Were in the battery would I find a burned wire?
If you can't afford a trained technician, I suggest you look at YouTube videos for how to uncover and inspect Gen 1 Prius HV battery. Yes you need to disconnect the 12V &HV batteries
